ERRONEOUS PRESCRIPTION
•Brand name PRECEDES Generic name
•Generic name is the one in PARENTHESIS
•Brand name is not IN PARENTHESIS
•One or More drug is prescribed in one prescription
WHAT TO DO WITH ERRENEOUS PRESCRIPTION
Shall be filed. Shall be kept and reported by the pharmacist of the drug outlet or any interested party to the nearest DOH office for appropriate action
VIOLATIVE PRESCRIPTION
•Generic name is NOT WRITTEN
•Generic name is written but NOT LEGIBLE and brand name is written and is LEGIBLE
•When the Brand name is indicated with a NO SUBSTITUTION phrase
WHAT TO DO WITH VIOLATIVE PRESCRIPTION
Shall not be filed. Shall be filed. Shall be kept and reported by the pharmacist of the drug outlet or any interested party to the nearest DOH office for appropriate action. The pharmacist should also advise the prescriber(doctor) of the problem and/or instruct the customer to get the proper prescription
IMPOSSIBLE PRESCRIPTION
•ONLY the Generic name is written but it is NOT LEGIBLE
•Generic name does NOT CORRESPONDS the Brand name
•BOTH Generic and Brand name is NOT LEGIBLE
•Drug product prescribed IS NOT FDA APPROVED
