What is erectile dysfunction?
Persistent inability to attain + maintain an erection sufficient for sexual intercourse
Causes of erectile dysfunction
Vasculogenic (HTN, CVD, DM)
Neurogenic (MS, Parkinsons)
Anatomical (Peyronie’s disease, hypospadius)
Hormonal (hypogonadism, thyroid issues, Cushings)
Psychogenic (depression)
Drugs (antihypertensive, antidepressants + APs)
RF for erectile dysfunction
Obesity, DM, dyslipidemia, metabolic syndrome, HTN, endothelial dysfunction, lack of exercise, smoking
What is associated with erectile dysfunction?
CVD
Management of erectile dysfunction
Lifestyle measures
PDE-5 inhibitors (sildenafil)
Pathology of hyposadias
Congenital anomaly or male urethra - ventral placement of urethral opening
Causes abnormal foreskin development + penile curvature
Workup for pts at risk of disorders of sexual development
Pelvic US, karyotype + serum electrolytes
What other features may indicate a pt is at risk of disorder of sexual development?
Cryptochordism
Hypospadias
Management of hypospadias
Surgery
No surgery if pt has forme fruste (incomplete hypospadias) that doesn’t affect voiding or sexual dysfunction
