palmar digital nerve block

A
  • clean the area with alcohol wipes
  • palpate the VAN bundle just proximal to the bulbs of the heels, and palpate the cartilage of the foot. where the bundle and the foot cartilage meet is where you want to inject
  • use a 25 guage short needle to inject in this location, DRAW back to make sure you are not in a vessel
  • inject 2-3mls of mepivicaine , then MASSAGE
    do two injections, one medial and one lateral
  • this blocks the palmar digital nerve, right at the level of P2 down

abaxial sesamoid nerve block

A
  • clean the area with alcohol wipes
  • palpate the VAN bundle at the level of the fetlock joint where you’d feel the digital pulses
  • use a 25 gauge needle to inject parallel to the nerve fairly superficial, DRAW back
  • inject 2-3mls of mepivicaine then MASSAGE
  • 2 injections, medial and a lateral
  • this blocks the dorsal and palmar braches of the digital nerve, at the level of the fetlock joint downwards

low 4 point block

  • clean the area with alcohol wipes
  • palpate the button of the splint bones and move just distal to that, this is your marker for blocking the palmar metacarpal nerve, want to inject perpendicular
  • move slightly proximal and separate the flexor tendons from the suspensory ligament, this is your marker for blocking the palmar nerve, inject perpendicular
  • use a 25 gauge short needle to inject, doing 4 injections, both medial and lateral for the 2 nerves, DRAW BACK
  • inject 2-3mls of mepivicaine into each location then MASSAGE

high 4 point block

  • clean the area with alcohol wipes
  • palpate the heads of the splint bones and move a few inches distal, and you want to inject behind them at an angle, this blocks the palmar metacarpal nerves
  • move slightly distal and inject between the flexor tendons and the suspensory ligament on each side, this blocks the palmar nerve
  • 4 injections using 25 gauge needle, DRAW BACK
  • 2-3 mls mepivicaine each site, MASSAGE
