Epithelialization Phase Flashcards
Periwound Skin
Acute Epithelialization Phase
3
Early phase is same as acute inflammation (unblanchable erythema, discolored skin, ecchymosis)
Normal ethnic color returns as inflammation subsides.
Hemosiderin staining if chronic wound.
Peirwound Skin
Chronic Epithelialization Phase
6
Same as chronic inflammatory or absence inflammatory.
Ischemic (pale) or ashen.
Purplish w dependency.
Dry, Flaky, (kyperkeratotic–due to desiccation or aging skin)
Maceration: pale, wrinkled, soft, thin.
Hemosiderin staining.
Periwound Skin
Absense of Epithelialization Phase
3
Scar tissue
Same as chronic or absence inflammatory.
Dry w/ hyperkeratosis or lipodermatosclerosis.
Wound Tissue
Acute Epithelialization Phase
3
Even with round edges
Pink/red granulation
Reduction in wound surface area
Wound Tissue
Chronic Epithelialization Phase
2
Not connected with wound edge
Hypergranulation
Wound Tissue
Absence of Epithelialization Phase
3
Absense of resurfacing from edges or dermal appendages.
Hypogranulation
Presence of scab or necrotic tissue.
Undermining/Tunneling
Acute Epithelialization Phase
1
Steep walls limit migration
Undermining/Tunneling
Chronic Epithelialization Phase
1
Steep walls limit migration
Undermining/Tunneling
Absence of Epithelialization Phase
1
Steep walls limit migration
Wound Edges
Acute Epithelialization Phase
3
New skin moves out from wound edge and dermal appendages in irregular pattern
Bright pink color, regardless of usual skin pigmentation.
Texture is soft to firm and flexible to touch, thin
Wound Edges
Chronic Epithelialization Phase
4
Epithelial ridge
Rolled under or thickend
Dry, flaky skin
Rounding off of wound shape
Wound Edges
Absense of Epithelialization Phase
4
Fibrotic wound ede
Rounding off of wound shape and edges.
Macerated.
Dry, flaky skin
Wound Drainage
Acute Epithelialization Phase
1
Minimal to scant serous or serosanguineous
Wound Drainage
Chronic Epithelialization Phase
1
Absent, dry, if hypergranulation, minimal/moderate
Wound Drainage
Absence of Epithelialization Phase

Absent, Dry
