Actions
Alpha effects - peripheral, coronary, renal, and mesenteric vasoconstriction and bronchial arteriole constriction
Beta effects - increased heart rate, myocardial contractility, automaticity of all pacemaker cells
Decreased chemical mediator release during allergic reaction
Lowers ventricular threshold
Class
Sympathomimetic/ Natural Catecholamine
Indications
Acute asthma attacks
severe allergic reaction
Anaphylactic shock
Cardio arrest
Contraindications
Narrow angle glaucoma
Hypovolemic shock
Hypertension
Dosage
Adult:
Cardiac - 1mg IV push of 1:10,000 solution every 3-5min
Asthma/ Allergic Rx/ Anaphylaxis - 0.3-0.5 mg SQ of 1:1,000 solution, may be repeated in 20min - in severe cases, 0.3-0.5mg 1:10,000 IV may be administered
Peds:
Cardiac arrest - 1st IV or IO dose is 0.01 mg/kg of a 1:10,000 solution. Repeat dose is 0.1 mg/kg 1:1,000 diluted w/5-10ml saline
Asthma/ Allergic Rx/ Anaphylaxis - 0.01 mg/kg SQ of 1:1,000 solution (max single dose 0.3mg), may be repeated in 20 min
- in severe allergic Rx 0.01 mg/kg may be administered IV push slowly
Side effects
Hypertension
Tachycardia
Hyperglycemia
Cerebral hemorrhage
Considerations
Caution in pregnancy due to decrease placental blood flow
May req repeat dose in 15-20 min when given SQ for asthma
Extreme caution in pts over 40 years old
