What is the purpose of centor criteria?
Used to confirm severity of infection, usually suspect Group A Streptoccus
What is the centor criteria?
3 or 4 out consider antibiotic treatment
- tonsillor exudate
- ABSENCE of cough
- History of fever
- Presence of anterior cervical lymphadenopathy
Treatment for tonsillitis with a centor criteria of 3-4?
Benzylpenicillin IV if unable to swallow
Penicillin if able to swallow
Treatment for tonsillitis with a centor criteria of 3-4 and PENICILLIN ALLERGY?
Clarithromycin IV or PO depending on if they are able to swallow
When should you considor antibiotics in cases of acute otitis media?
<2 years and bilateral AOM
bulging membrane
perforated membrane
Antibiotic of choice for acute otitis media?
Amoxicillin for 5 days
Antibiotic of choice for acute otitis media with PENICLLIN ALLERGY?
Clarithromycin for 5 days
Treatment for MILD otitis externa?
Acetic acid for 7 days
Treatment for MODERATE otitis externa?
Otomize or Sofradex
These are a combination of dexamethazone, antibiotics and acetic acid topical sprays/drops
When should an ear be swabbed for culture in cases of otitis externa?
If not responding to one course of treatment of topical therapy
Treatment for fungal infection in otitis externa?
clotrimazole topical drops
Treatment for bacterial infection in otitis externa?
Topical drops of gentamicin or ciprofloxacin as per sensitivities
Which organisms are sensitive to gentamicin?
Gram negatives
Which organisms are sensitive to ciprofloxacin?
Gram negatives
Which organisms commonly cause acute otitis media and why?
Streptococcus groups, haemophilus influenzae.
Common URTI, travel along eustachian tube to infect middle ear.
