ENT: normal
Ears: Ear canals are clear and non-erythematous. Tympanic membranes are normal in appearance with normal landmarks and cone of light. No bulging or retraction noted. Hearing is grossly intact.
Nose: Nose reveals no obstruction or discharge. Nasal septum is midline. No external lesions, mucosa non-inflammed, turbinates normal.
Throat: Oropharynx reveals no inflammation, swelling, exudate or lesion. Oral mucosa is pink and moist with good dentition.
ENT: sinusitis
Head: Normocephalic/atraumatic. Symmetric. Normal hair distribution and pattern.
Eyes: PERRLA. EOMI (extra-ocular motion intact). Conjunctiva pink. Sclera white. No redness or drainage noted.
Ears: Tympanic membrane intact, dull and full bilaterally. Cone of light noted. Ear canals clear bilaterally.
Nose: Nasal mucosa reddened and moist. Septum midline. Nasal turbinates edematous, no obstruction. Thick and purulent drainage noted. Frontal and maxillary sinus tenderness. Throat: Oral mucosa pink and moist. Tongue midline. Pharynx reddened, without exudate. Tonsils 1+. Good dentition
ENT: acute otitis media
Bulging, opaque, erythematous, and immobile TM.
ENT: otitis externa
Otalgia with pinna manipulation. Ear canal edema and erythema with thick seropurulent otorrhea
ENT: ROS
No vertigo, no vision changes, no eye pain. No regurgitation/sour taste in back of throat
c/o runny/stuffy nose
mild sore throat
fever
cough/dry
ongoing for past 1 week
better but cont to have cough/ no wheezing/ dry
General
Well appearing, NAD
Mild nasal d/c turbs swollen, pharynx no erythema, TM n b/l
