Endoscopy Lo's Flashcards
1
Q
What is an endoscope?
A
long thin tube with a small camera inside. it is inserted directly into the organ or cavity
2
Q
Examples of types of endoscopy and what they look at
A
- Arthroscopy–> joints
- bronchoscopy–> trachea and bronchi
- colonoscopy–> colon+ large intestine
- colposcope–> vagina + cervix
- laparoscope–> abdominal organ
- neuroendoscope–> area of brain
- sigmoidoscopy–> sigmoid colon
- thoroscope–> pleura
3
Q
name 5 GI endoscope techniques
A
- sigmoidoscopy- rectum + sigmoid colon in via anus
- enteroscopy- allows better visiulaisation of small bowel than EGD
- Endoscopic retrograde cholangiopancreatography (ERCP)- ducts, involving liver, pancreas and gallbladder
- endoscopic ultrasound- used to look at histological layers of GI btract and surrounding organs
- percutaenous endoscopic gastrotomy- flexible feeding tube is placed with the assistance of an endoscope through a small incision in the abdominal wall into the stomach.
4
Q
sedation options available for upper and lower GI endoscopy
A
- 1st choice–. propafol- hypnotic
- opiates–> meripiridine, fentanyl
- benzodiazepines-> midazolam, diazepam
5
Q
patient experience of endoscopy
A
- cramps
- twinges
- painful
- stressful
- unnatural
6
Q
complications of using an endoscope
A
- perforation
- reaction to sedation
- infection
- bleeding
- pancreatitis from ERCP
- bloating
- numb throat
7
Q
Management of upper GI bleed
A
pre endoscopic mamagement
- haemodynamic assessment + resuscitation
- blood transfusion–> Hb to 70/80 g/L
- risk assess blled–> Blatchford score
- erythromycin (prokinetic) –> PPI to be cosnidered
endoscopic urgent if haemodynamically ustable
- injection therapy (epinephrine)
- thermal probes (electro coagulation, heater probe)
- oesophageal varices? ligation/ balloon tamponade
- gastric adhesion- tissue adhesion
Post endoscope
- PPI for ulcers + aspirin
- cirrhosis- antibiotics
8
Q
What is a PEG
A
PEG= percutaneous endoscopic gastrotomy
endoscopic medical procedure in which a PEG tube is fed into the stomach via the anterior abdomen. Allows for feeding to bypass oesophagus
