A

Abnormal vaginal bleeding i.e. post-menopausal bleeding

A

  • HRT
  • Vaginitis

How would we investigate a case of abnormal vaginal bleeding?

A
  1. Pelvic and speculum exam
  2. Transvaginal US
  3. Endometrial biopsy or hysteroscopy (confirm diagnosis by histology)

What signs would you look for on Transvaginal US?

A

Endometrial thickening (>4mm) and irregular contour would suggest cancer

A

Total Abdominal Hysterectomy and Bilateral Salpingo-oophorectomy with peritoneal washings

(Removal of uterus, cevix, ovaries and fallopian tubes)

How do we stage endometrial cancer?

A

With the FIGO classification:

  1. In myometrium
  2. In cervix
  3. a - In serosa, b - vagina, c - para-aortic nodes
  4. In other pelvic organs (bladder, bowel, inguinal nodes)

What does a transvaginal US look at?

A
  • Measure endometrial thickness
  • Looks at the endometrial contour
  • Should do biopsy if >4mm or irregular
