Sync. Cardioversion
Use: Tachydysrhythmia
Dose:
Peds: Start 0.5-1 J/kg, Increase to 2 J/kg if needed
Adult:
Narrow regular: 50-100 J
Narrow irregular: 120-200 J biphasic (200 J monophasic)
Wide regular: 100 J
Wide irregular: defibrillation dose (not synchronized)
Dantrolene IV
Use: Malignant hyperthermia
Dose: 2.5 mg/kg IV prn, up to 10 mg/kg
Atropine IV
Use: Hypotension, bradycardia
Dose:
Adults: 1 mg (MAX 3 mg total)
Peds: 0.02 mg/kg/dose IV (MAX: 0.5 mg/dose)
Succinylcholine IM
Use: Laryngospasm, ventilation & oxygenation
Dose: 4 mg/kg IM, MAX: 100mg
Methylprednisolone IV
Use: Allergic reaction
Dose: 2mg/kg IV
Phenylephrine IV
Use: Hypotension, PSVT
Dose:
Adult: 50-100 mcg bolus; titrate 10-20 mcg/min
Peds: 0.1 mg/kg SC/IM, MAX: 5 mg/dose IM
Ondansetron IV
Use: Nausea / vomiting
Dose: 0.1 mg/kg IV, MAX: 4mg
Racemic Epinephrine
Use: Suggest non-rebreather nebulizer
Dose: 0.05 mL/kg (Max 0.5mL) of 2.25% solution
Succinylcholine IV
Use: Chest wall rigidity, laryngospasm
Dose:
Peds: 0.5 mg/kg IV, MAX: 20 mg/dose
Adult: 20+ mg IV up to 0.5 mg/kg
Epinephrine IV (Pre-puberty/Cardiac Arrest)
Use: Cardiac arrest
Dose:
Pre-puberty: 0.01 mg/kg IV push q2m, MAX: 1 mg
Adult: 1 mg IV push
Nitroglycerin PO (Adult)
Use: Chest pain, hypertension
Dose:
Spray: 1-2 SL q5m prn (0.4mg per actuation)
Tab: 0.4mg SL q5m, MAX: 3 doses within 15 min
50% Dextrose IV
Use: Hypoglycemia
Dose: 0.5 g/kg IV, MAX: 25 grams
Adenosine IV
Use: Tachycardia
Dose:
1st dose: 0.1 mg/kg IV, MAX: 6 mg
2nd dose: 0.2 mg/kg IV, MAX: 12 mg
Amiodarone IV (Tachycardia)
Use: Tachycardia
Dose:
Peds & Adult: 5 mg/kg IV over 10 minutes, MAX: 150 mg

Oxygen
Use: Chest pain
Dose: Titrate to ≥ 94% SpO2 for <94% SpO2
Diltiazem IV (Adult only)
Use: Tachycardia
Dose: 1st dose: 5 mg IV over ten minutes
