Emergency hormonal contraception = ‘morning after pill’
- Levonorgestrel → synthetic progesterone (trade name: Levonelle One Step)
- Uripristal acetate → progesterone receptor modulator (Ella One)
MoA of Levonorgestrel
(as emergency contraception)
Levonorgestrel = synthetic progesterone (Levonelle One Step)
MoA:
- delays ovulation for 5-7 days → after this time sperm will become non-viable
- licensed for use within 72 hours of unprotected sex
How may hours after unprotected sex can Levonorgestrel (Ella One Step) be used?

MoA of Ulipristal acetate (EllaOne)
Ulipristal acetate = progesterone receptor modulator
MoA:
- Delays ovulation for 5-7 days → sperm becomes non-viable
- Licensed for use within 120 hours of unprotected sex

Does emergency hormonal contraception have an effect on implantation?
- Both emergency pills (Levonorgestrel and Ulipristal acetate) have no known effect on disruption/inhibition of implantation
- Their effect is due to inhibition of ovulation

Contraindicaitons for levonorgestrel as an emergency contraception
There are no absolute contraindications, but the efficacy may be reduced by:
- diseases of malabsorption e.g. Crohn’s
- enzyme- inducing drugs e.g. rifampicin,
What to do if a patient on enzyme-inducing drug refuses IUD and wants to use Levonorgestrel?
Efficiacy of Levonorgestrel will be reduced → use double dose (3 mg at once may be taken)
Contraindications for use of Ulipristal Acetate
- Diseases of malabsorption e.g. Crohn’s
- Hypersensitivity to Ulipristal Acetate
- Severe hepatic dysfunction
- Enzyme inducing drugs e.g. rifampicin
- Give 3mg levonorgestrel if the patient refuses an IUD, ulipristal acetate is absolutely contraindicated here
- Breastfeeding – avoid breastfeeding for 7 days after taking UPA
- Asthma insufficiently controlled by corticosteroids
- Drugs increasing gastric pH e.g. omeprazole, ranitidine
Contraindications for IUD as a form of contraception
- Uterine fibroids with distortion of the uterine cavity
- Documented or suspected pelvic inflammatory disease (PID)
- Documented or suspected STI (especially chlamydia or gonorrhoea)
Advice patient to seek help after receiving hormonal emergency contraception if…
Vomiting occurs within:
- 2 hours of taking Levonorgestrel
- 3 hours of taking Ulipristal acetate
*this is because medication may not have been absorbed
