A
plan A: (face mask ventilation and tracheal intubation optimise head and neck position pre oxygenate adequate neuromuscular blockade direct/ video laryngoscopy (max 3 + 1 attempts) external laryngeal manipulation bougie remove cricoid pressure maintain oxygen and anaesthesia
(confirm with capnography)
FAILED INTUBATION
plan B: (maintaining oxygenation, SAD insertion)
second generation device recommended
change device or size (max 3 + 1 attempt)
oxygenate and ventilate
(stop and think, consider risks and benefits.
- wake patient up
- intubate via SAD
- proceed without intubation
- trachyostomy or cricothyroidotomy
FAILED SAD INSERTION
plan C: (face mask ventilation)
if impossible, paralyse.
final attempt and face mask ventilation
use two person technique and adjuncts
(wake patient up)
DECLARE CICO
plan D: (emergency front of neck access)
scalpel cricothyroidotomy
