Electrolytes Flashcards
Hypernatremia: Causes
- High water losses
- Excessive intake (rare)
Hypernatremia: Signs
Lethargy, seizures, coma
Hypernatremia: Treatment
Increased fluids and/or restrict Na
Hypernatremia: Lab Value Classification
Na >150 mEq/L
Hypernatremia: Correction Course
Correct over days due to change in tonicity of the brain tissue
Hyponatremia: Causes
- Excess free water
- Increase Na losses/inadequate Na intake:
Renal dysplasia
Hypoxia
Resp Dis
Acute tubular necrosis
Polycythemia
Increased Fluid and salt intake
Use of theophylline of diuretics
Hyponatremia: Signs
Lethargy, seizures, coma
Hyponatremia: Treatment
Restrict fluids and/or Na supplements
Hyponatremia: Lab Value Classification
<130 mEq/L
Hypokalemia: Lab Value Classification
K <3.5 mEq/L
Hypokalemia: Causes
Diuretics, NG losses, diarrhea, renal dysfunction
Hypokalemia: Signs
EKG changes (flat T waves, prolonged QT, U waves), arrhythmias, ileus, lethargy
Hypokalemia: Treatment
SLOWLY CORRECT IV or orally
Hyperkalemia: Lab Value Classification
K >6 mEq/L
Hyperkalemia: Causes
Iatrogenic, severe acidosis, acute renal failure, RBC breakdown s/p transfusion,, Congenital Adrenal Hyperplasia (CAH)
Hyperkalemia: Signs
EKG changes (peaked, T waves, wide QRS, brady/tachycardia, SVT, V tach, V fib), arrhythmias death
Hyperkalemia: Treatment
D/C all potassium supplements, give calcium gluconate, sodium bicarbonate, albuterol, insulin, Lasix, dialysis/exchange
Hypocalcemia: Lab Value Classifications
Ca <7 mg/dL
iCa <1 mg/dL
Hypocalcemia: Causes
Prematurity, IUGR, IDM, HIE, Hypoparathyroidism, Vit D deficiency
Hypocalcemia: Signs
Asymptomatic, jitteriness, irritability, seizures
Hypocalcemia: Treatment
BASED ON iCA LEVEL
Observation, repletion (Ca gluconate), or supplementation
Hypercalcemia: Lab Value Classifications
Ca >11 mg/dL
iCa >5
Hypercalcemia: Causes
Rare in neonates, usually associated with HMF or iatrogenic
Hypermagnesemia: Lab Value Classification
Mg >2.3 mEq/L
Hypermagnesemia: Causes
Maternal treatment with magnesium for PIH or tocolysis
Hypermagnesemia: Signs
Respiratory depression, apnea, hypotonia, decreased GI motility
Hypermagnesemia: Treatment
Self-limited resolves within a few days
