Electrolyte Replacement Flashcards
No replacement needed if K+ >/= _____ ?
> /= 4.6
Custom replacement (only if specified) if K+ ____ - ____?
4.1 - 4.5
Less aggressive replacement for K+ ____ - _____?
What does this mean?
3.6 - 4
Full replacement = 20 mEq
Half replacement = 10 mEq
*Recheck levels with AM labs
More aggressive replacement for K+ = _____ ?
What does this mean?
= 3.5 Full replacement = 40 mEq Half replacement = 20 mEq *Recheck levels 1 hr after IV replacement or 4 hrs after PO/FT replacement
Can K+ tab be crushed?
No. K+ tab is extended release. DO NOT CRUSH.
Max infusion for K+ (central v. peripheral line)
Central line: 20mEq / 100mL over 1 hr
Peripheral: 10mEq / 100mL over 1 hr
note: 20mEq/50mL is possible per MD order but still give no faster than 20mEq/hr.
No replacement needed if Mg+ >/= _____ ?
> /= 2 (No replacement)
If Mg+ 1.8 – 1.9 replace ___? recheck___?
Full IV: 2 grams IVPB
Half IV: 1 gram IVPB
PO/FT: 400 mg PO/FT q 6 hrs x 2 doses
*Recheck w/ AM labs
If Mg+ 1.6 – 1.7 replace ____? recheck ___?
Full IV: 4 grams IVPB
Half IV: 2 gram IVPB
PO/FT: 400 mg PO/FT q 6 hrs x 2 doses
*Recheck w/ AM labs
If Mg+ 1.1 – 1.5 replace ___? recheck___?
Full IV: 6 grams IVPB
Half IV: 3 gram IVPB
PO/FT: Call MD to get an order for IV replacement
*Recheck levels 2 hrs after IV replacement
If Mg+ 1 or below: Replace __? Recheck__?
Full IV: 8 grams IVPB
Half IV: 4 gram IVPB
PO/FT: Call MD to get an order for IV replacement
*Recheck levels 2 hrs after IV replacement
Standard IV Infusion rate for Mg+?
Emergency max rate?
