- Must give as soon as this is found out

What happens if I go over 30 minutes from original test?
Document the reason in the remarks
I.e. 15 minute wait instructions not given until “whatever time”
EBT Methodology Types
- Electrochemical oxidation or fuel cell
- Infrared spectrometry
- Gas chromatograph
Reasons for cancelled tests
- Test number result displayed not the same as test number or result printed out
- If confirmation test done before 15 minute wait period is completed
- No air blank done before confirmation test or result of blank is not zero
- EBT does not print confirmation test results
- If accuracy check fails
- If test is uncompleted
What are some examples of correctable errors?
- Didn’t sign step 3 (BAT)
- Employee refuses to sign step 4 and you don’t put in remarks
- If you use a non-DOT form
How do I correct…..
Didn’t sign step 3 (BAT)
Supply a written memorandum that the specific test in question was performed by you and in accordance with the regulation.
A copy goes to employee, DER and your file

Employee refuses to sign step 4 and you don’t put it in the remarks
Supply a written memorandum that employee refused to sign and you did not note it in remarks
Copy goes to employee, DER and your file

If I use a non-DOT form
Supply a written memorandum why you used the wrong form and also note what steps you are going to do to prevent this from happening again.
