missed period
constant lower abdominal pain in the right or left iliac fossa
vaginal bleeding
lower abdominal or pelvic tenderness
cervical motion tenderness
dizziness or syncope (blood loss)
shoulder tip pain (peritonitis)
What are the transvaginal US findings in ectopic pregnancy?
gestational sac containing a yolk sac or fetal pole may be seen in a fallopian tube - tubal ectopic pregnancy moves separately from the ovary, corpus luteum will move with the ovary
empty uterus
fluid in the uterus
What is a pregnancy of unknown location?
positive pregnancy test but there is no evidence of pregnancy on US
What does a hCG rise of >63% after 48 hours indicate?
intrauterine pregnancy
When should a pregnancy be visible on US?
hCG level about 1500
What does a hCG rise of <63% after 48 hours indicate?
ectopic pregnancy
What does a hCG fall of >50% after 48 hours indicate?
miscarriage
What are the management options for an ectopic pregnancy?
requires termination as not a viable pregnancy:
expectant management (awaiting natural termination)
medical management = methotrexate
surgical management
What are the criteria for expectant management of an ectopic pregnancy?
follow up needs to be possible to ensure successful termination
ectopic needs to be unruptured
adnexal mass <35mm
no visible heartbeat
no significant pain
hCG level <1500 IU/l
What is the criteria for medical management of an ectopic pregnancy?
follow up needs to be possible to ensure successful termination
ectopic needs to be unruptured

How long after having an ectopic pregnancy terminated with methotrexate are women advised to wait before getting pregnant?
3 months (highly teratogenic)

vaginal bleeding
nausea and vomiting
abdominal pain
stomatitis (inflammation of the mouth)

What is a missed miscarriage?
fetus is no longer alive but no symptoms have occurred
What is a threatened miscarriage?
vaginal bleeding with a closed cervix and a fetus that is alive
What is an inevitable miscarriage?
vaginal bleeding with an open cervix
What is an incomplete miscarriage?
retained products of conception remain in the uterus after the miscarriage
