NOSE focused assessment

A

inspect and palpate nose for symmetry and drainage, report findings
-test patency of nostrils (assessing for obstruction with pen light)
-inspect nasal septim (midline) report findings

MOUTH focused assessment

A

universal precautions
-inspect and palpate lips for color, moisture, cracking, lesions, tenderness
-inspect tongue (midline)
-Palpate tongue for tenderness/masses
-inspect ventral surface of tongue (touch roof of mouth and move side to side)
PENLIGHT:
-inspect teeth and gums, note number, color, restoration
-note color of gums. check for swelling/bleeding. report findings
-inspect buccal mucosa for color and condition
-ask person to say ahhh and note the soft palate and uvula rise at midline
-inspect pharyngeal wall, describe
-inspect tonsils for size, exudate, grade
-verbilize testing of gag reflex
-note any halitosis

EARS assessment

A

-inspect external ear for skin condition, size, symmetry, shape. report
-identify and palpate external ear structures (auricle or pinns, tragus, lobule, helix, mastoid process, external auditory meatus) for masses, lesions, tenderness
-use a penlight and retract adult ear up and back. inspect auditory meatus for foreign bodies, discharge, cerumen (describe if found)
-test hearing acuity by the whisper voice test. assess ccranial nerve 8 findings.
