What forms can it take?
Acute allergen exposure in sensitised pts
Chronic exposure = granuloma formation and obliterative bronchiolitis
Name 3 causes
Bird fancier’s lung: avian protein in bird droppings
Farmer’s/mushroom workers
Malt worker’s lung: aspergillus clavatus
What are the acute SX? (4-6h post exposure)
Fever, rigors
Dry cough
Dyspnoea
What are the chronic SX?
Increasing dyspnoea
Wt loss
T1RF
Cor pulmonale
What IX are required?
Bloods: show no eisinophilia
CXR
Spirometry
BAL
What do you see on CXR?
Mid/upper zone fibrosis - honeycomb lung
What pattern is seen on spirometry?
Restrictive
What is seen on BAL?
Increased lymphocytes and mast cells
How is it MX?
Avoid exposure
Steroids: acute/ long-term
?compensation
