What are the medicines and conditions that can cause xerostomia?
antimuscarinics antihistamines TCAs diuretics irradication of head and neck cancer dehydration anxiety sjogrens syndrome
What is the management for xerostomia?
1) manage underlying cause of dehydration
2) frequent sips of cold unseated drinks, suck on ice or sugar free fruit pastilles, chew sugar free gum
3) artificial saliva substitute
How long does it take pilocarpine tablets to work?
up to 3 months
What are the complications of dry mouth?
dental caries
periodontal disease
candidiasis
What are the side effects of chlorhexidine mouthwash?
- dry mouth
- brown/yellow stains on tongue and teeth
- taste altered
Can you use chlorhexidine mouthwash and toothpaste together?
No. They are incompatible. Wait 30 mins before each and rinse thoroughly with water inbetween
What is the MOA of hydrogen peroxide mouthwash?
oxidising agent
What nutritional deficiencies can cause oral ulceration and inflammation?
- iron
- folic acid
- vitamin b12
When do we refer mouth ulcers urgently?
If unexplained mouth ulcer for more than 3 weeks
What is the first line treatment for mod ulcers?
topical corticosteroids like buccal hydrocortisone
When do we avoid lidocaine?
Near meals to avoid choking
Which type of water is best for ulcers?
tepid (lukewarm)
What are the contraindications to flubiprofen lozenge?
If allergic to NSAIDs/Hx of asthma attacks precipitated by NSAIDs
What are the contraindications to choline salicylate gel?
In children <16yrs due to the risk of Reyes syndrome
What is the dose and counselling for Nystatin suspension?
100000 units QDS for 7 days. Continue for 48 hours after lesions have resolved. Use after food, use pipette and hold in mouth for as long as possible before swallowing/spitting
