  1. BMI: >30kg/m2 and at least 3 months of supervised diet, exercise and behaviour modification fails to reach realistic weight loss
  2. BMI: >28kg/m2 with risk factors e.g. diabetes, heart disease, HTN, obstructive sleep apnoea
