Nicardipine for Stroke

A

Mix 25 with 240, to make 250 mL

Start at 5 mg/hour, increase by 2.5 mg q15min max 15 mg/hour.

A

20 mg IV over 2 minutes,
repeat 40 or 80 q 10 mins to 300 mg total.

5-20 mg IV, repeated every 5-10 minutes

Peak: 20-40 minutes
Half-life: 3-6 hours

Esmolol for Aortic Dissection

A

Loading 500 mcg/kg over one minute
Then 50 mcg/kg/min infusion.
Repeat procedure with bolus every 5 minutes,
increasing the infusion by 50 mcg/kg/minute,
max 200 mcg/kg/minute.
Peak 5 minutes.
HL 9 minutes.
