Mix 25 with 240, to make 250 mL

Start at 5 mg/hour, increase by 2.5 mg q15min max 15 mg/hour.

20 mg IV over 2 minutes,
repeat 40 or 80 q 10 mins to 300 mg total.

5-20 mg IV, repeated every 5-10 minutes

Peak: 20-40 minutes
Half-life: 3-6 hours

Initiate 0.25 mcg/kg/min, increase by 0.25 every q3-5min to a max of 10.

Vasopressin for GI bleeding

500 mcg per 250 mL NS

25 to 50 mcg per hour

8 mg per hour infusion

Esmolol for Aortic Dissection

Loading 500 mcg/kg over one minute
Then 50 mcg/kg/min infusion.
Repeat procedure with bolus every 5 minutes,
increasing the infusion by 50 mcg/kg/minute,
max 200 mcg/kg/minute.
Peak 5 minutes.
HL 9 minutes.
