SKIN: Isotretinoin (Accutane)
(retinoid)
-for severe cystic acne
-MOA: inhibit sebaceous gland, antikeratinizing, anti-inflammatory
-PO
accutane adverse
-common: nosebleeds, inflamed lips/eyes, dry or itch skin/nose/mouth
-avoid sun
-rare: depression
-can impact liver: get LFTs
accutane contraindications and education
-pregnancy category X
(rule it out before start and test every month)
(be on 2 forms of birth control 1 month before start, during, and after)
-hazadorous drug: safe handling
Topical administration
-to prevent systemic absorption, DONT apply on:
-large abraded/denuded areas
-medicated skin covered with occlusive dressing
-solvent where drug is added
-no apply heat
-wear gloves, minimal exposure
-monitor for severe itching, swelling, burning, open blisters
EYES: Timolol (Timoptic)
-treat glaucoma –> MOA: decrease production of aqueous humor to lower IOP
-WILL need to take it lifelong
-is a non selective beta blocker
-common adverse: stinging on application, bronchoconstriction and drop BP, decrease BG
-safe admin: light pressure to lacrimal sac 1min
-bc nonselective: monitor BP/HR, BG levels
-HOLD: if COPD/asthma, diabetic, hypotensive
