9
Q

Zetia action

A

Acts at the brush border of the small intestines and inhibits the absorption of cholesterol decreasing delivery of intestinal cholesterol to the liver. This causes a reduction in hepatic cholesterol stores, and an increase in clearance of cholesterol from the blood.

16
Q

When should Questran and welchol
Be taken

A

Take with food at largest meal 1 to 4 times a day

It is available as a powder, granules and tablets. It can be mixed with hot water, applesauce, but not carbonated drinks thyroid meds, anabiotic’s vitamins, be taken separately to prevent interaction in decreasing bioavailability.

17
Q

Niacin, what is it?

A

Naturally occurring the vitamin can improve cholesterol levels when use it does is 100 to 300 times the recommended daily vitamin allowance

18
Q

Niacin action

A

Decreases VLDL synthesis in the liver inhibits Lipo lysis in adipose tissue and increases lipoprotein lipase activity

19
Q

Niacin dosage

A

At least 1.5 g niacin daily titrated gradually.

20
Q

Niacin contraindications

A

Hepatic dysfunction, severe, hypertension, persistent, hyperglycemia, acute gout, new onset, a fib, or cute peptic ulcers

21
Q

Niacin side effects

A

Flushing itching take 325 mg of aspirin 30 minutes prior G.I.

22
Q

What labs you draw a niacin

A

Draw LFTs at six and 12 weeks and dose change to monitor

23
Q

Does niacin work?

A

Niacin therapy does not lead to significant reduction in total or case, specific mortality, or recurrent cardiovascular events among people with or at risk of atherosclerotic cardiovascular disease a.k.a. it doesn’t work

24
Q

What are Fibric acid derivatives?

A

Gemfibeozil loped ,fenofibrates tricor , fenofibric acid trilipix

25
Q

What are fibric acid derivatives?

A

A class of lipid, lowering drugs, mainly affect, triglyceride and HDL cholesterol levels

26
Q

Fibric acid derivatives action

A

Simulation of lipoprotein lipase, which enhances the breakdown of the LDL to LDL cholesterol

27
Q

Fibric dosage

A

Gemfibrozil given in 600 mg doses twice daily with breakfast and dinner

TriCor 145 mg daily, mixed hyperlipidemia
Tri core 48 to 145 mg hypertriglyceridemia

28
Q

Fibric acid derivatives contraindications

A

Gallstone satanic renal dysfunction

29
Q

Fibric acid derivative side effects

A

G.I., myopathy, when combined with statins, niacin

Monitor at six and 12 weeks and periodically there after

Do not give these with statins

30
Q

What are prescription fish oils?

A

Lovaza, vascepa

31
Q

What is lovaza

A

Omega-3 ethyl esters
Decrease TG increase LDL

Dose is 4 mg four capsules per day

Can be taken with statins

32
Q

What is Vascepa

A

Lcosapent ethyl

Decrease TG less then LOVAZA

4 g four capsules per day

33
Q

What two options can treat high triglyceride levels over 500 mg

A

LOVAZA, VASCEPA

34
Q

What are adenosine triphosphate citrate lyase inhibitors

A

They are the newest class of LDL cholesterol in medication on the market

It is an enzyme of HMG-coa reductase in the cholesterol synthesis pathway

Inhibition of HCl reduces cholesterol biosynthesis in the liver and subsequently decreases LDL cholesterol by a regulation of LDL receptors ……. It increases LDL receptors in the liver so that the liver breaks down more fat.

35
Q

Medication for Familia homozygous hyper cholesterolemia

A

MIPOMERSEN, LOMITAPID (severe liver, toxicity side effects)

ALRROCUMAB (PRALUENT) and EVOLOCUNAB(Repatha) are monoclonal antibodies at target PCSK9

These medication’s are usually sub queue

36
Q

How high is a patient’s LDL with homozygous hyper cholesterolemia

A

LDL cholesterol levels greater than 600 at an early age

37
Q

What medication’s are used for combination therapy for hyperlipidemia

A

Lovastatin/niacin (Advicor)

Simvastatin/niacin (Simcor)

Combining sentence with a cholesterol absorption, inhibitor as benefits and reducing LDL cholesterol and ASCVD events

38
Q

What medication can bile acid resins be used with?

A

Statins niacin Fibric acid derivatives to enhance LDL cholesterol lowering

39
Q

Atherosclerotic, cardiovascular disease, risk, enhancing factors

A

Chronic kidney disease, family, history of premature, inflammatory diseases, metabolic syndrome, persistent, elevated cholesterol, LDL greater than 160 non-HDL 190, 219, south, Asian ancestry, preeclampsia or premature menopause before age 40, elevated biomarkers HSCRP greater than two
