What is the drug tariff?
Details the community Pharmacy contract with the NHS - essential, advanced, enhanced services.
A list of prices/drugs /appliances and fees for Pharmacies
List the 5 ways that community Pharmacies are paid
- Establishment and practice payment
- IT and SCR payments
- Fees (dispensing)
- Advanced services payments
- Enhanced services payments
What do establish and practice payments depend on?
Number of items dispensed per month
Describe fees as a pharmacy payment
NHS payment to Pharmacies for dispensing an item = £1.26 plus additional payments for other services claimed via endorsements or automatic payments
What are the payments from advanced services?
MUR = £28, up to £400 a year
NMS = £20.28, depends on how many completed a month
Flu £9.48
NUMSAS depends on cost of drugs and service provided but £10 consultation, 2.50 admin, 1.50 supply
Why are enhanced services payments not included in the drug tariff?
They are locally commissioned and paid for by councils, NHS England area team or CCG
What is the black list?
Items which are:
- unnecessary
- Expensive
- No medical use
- Duplication of other medicinal products
What items are allowed on FP10 prescription?
Any medicine, food, drink, medical cosmetic item as long as it is not in the black list
Where is the black list in the DT?
Part XVIIIA
How much is the patient charged for a Rx?
£8.80 - one charge per item
How is elastic hosiery charged to the patient?
One charge per leg
How is the patient charged when there are different strengths/flavours of the same item on FP10?
One charge
How is the patient charged if there are different formulations of the same item e.g slow release prednisolone Vs immediate release?
2 charges
How is a patient charged for combination packs?
One charge PER ITEM in the same combination pack
How are contraceptive items charged to patient?
no charge - except cyprindiol if not endorsed in the body of the Rx with OC
How should a prescription be endorsed/when?
- If there is >1 pack size
- Quantity supplied is defined by the number of treatment days on Rx
- Brand name, pack size and price if NOT in part VIIIA
What is SP in endorsement?
Special
What does ED stand for in an endorsement?
Extemporaneously dispensed
What does PD(n) stand for?
Packed doses for supervised consumption of methadone
What does MF stand for?
Measured and fitted e.g for elastic compression hosiery (part IXA)
- Trusses part IXA
- Para stomal garments Part IXC where size not indicated in DT
What is a special?
Unlicensed medicine that is ordered on a Rx for a named patient, produced by a specials manufacturer which is licensed and inspected by the MHRA
What is an unlicensed imported medicine?
Medicine that is unlicensed in the UK but licensed elsewhere in the world - imported by a MHRA licensed and inspected importer
How should an unlicensed medicine be endorsed?
All:
- SP
- Quantity or volume supplied
- Pack size
If the unlicensed medicine is not in Part VIIIB, what other endorsements should be included?
Price per pack
Number of packs
Manufacturer/importer license number
Batch number
What 5 records should be kept for items from part VIIIB for inspection by the licensing authority?
(keep for 5 years)
- Source
- Who it was supplied to
- Prescribers details
- Date of supply
- Quantity of supply
- Batch number
What 5 things must the Pharmacist endorse the COA, COC or invoice of the unlicensed specials/imported medicines with?
- Pharmacy stamp
- Date
- Pharmacist initials
- Prescriber details
- Invoice price less discount
Where and when must the endorsed COA/COC/invoice be sent?
At month end - must send a copy to the NHS england area team of the prescriber - removing patient details
List 3 things that receive payment for measuring and fitting if claimed using MF in the endorsement margin?
- Elastic and compression hosiery if in part IXA
- Trusses part IXA
- Para stomal garments if part IXC
What are Compression stockings?
Elastic leg garments which compress the limb, reduce the diameter of distended veins and increase venous blood flow - legs only
What does compression therapy prevent?
Reducing venous pressure, preventing formation of blood clots
Aids treatment of lower leg ulcers
Relieves aching heavy legs
When should stockings be measured?
In the morning whilst sitting for BK, standing for TL.
How should elastic/compression hosiery be endorsed?
Quantity Type Fabric Knit (MF if appt)
How should the patient be charged for elastic/compression hosiery?
Charges per leg £8.80 x number of articles
How are lymphoedema garments charged to patient?
Can you use MF?
charge to patient per TYPE of item
- Cannot use MF endorsements for these
What are trusses?
Framework - support hernia - padded belt e.g double scrotal elastic band truss
What are the details needed on an FP10 for urinary catheters?
Quantity
Size
Balloon size
Preferably brand, if Male or Female
What is included in part XIB of the DT?
Urinary catheters
Dressings
What is in IXB and IXC of DT?
IXB . = urinary incontinence not catheters
IXC = Colostomy bags etc
How much are patients charged for home delivery of items IXB and IXC?
All items get a £3.40 fee for home delivery - no endorsement needed
What is in part IXR of DT?
Chemical reagents e.g strips for detection of glucose /ketones = diabetics, INR test strips for warfarin
- ALL special containers
What are the 4 occasions that prescriptions arent placed in red separators to send off?
- expensive
- specials & unlicensed imports
- items with BB or OOP
- Hand written ammendments where the prescribers signature enroaches over an item on the Rx
