Heparin
PTT (60-70) aPTT (30-40) and per hospital protocol
Phenytoin
10-20 mcg/mL
Warfarin
PT/INR 2-3 (Watch out for bleeding)
Lithium
0.5-1.5 mEq/L (Watch out for dehydration)
Digoxin
0.5-2 ng/mL
Peaks and troughs
Draw peak level within on two hours after completed medication administration. Draw trough level right before (less than 30 minutes) the medication is administered.
Antidotes
Warfarin overdose: Vitamin K (With a high PT/INR level)
Opioid overdose Naloxone (Can reverse opioid overdose)
Electrolyte Levels
Potassium levels : 3.5 – 5.3 mEq/L With hypokalemia (low Potassium give Potassium Chloride) with too much potassium (hyperkalemia) give Kayexalate. Furosemide (a potassium- wasting diuretic may cause hypokalemia).
