Warfarin
Yellow book to record INR
Dose should be taken at same time every day
Women must not get pregnant (teratogenic)
Seek medical advice before diet change, limit alcohol.
Most common and important side effect is bleeding.
Monitor INR at regular intervals
Alendronic Acid (Bisphosphonates) - Directions for Administration
Empty stomach
70mg once a week
Same day each wek
Tablets swallowed whole with lots of water 30 minutes before breakfast
Sit/stand for 30 minutes after taking
Alendronic Acid (Bisphosphonates) - Side effects
Upper GI - dyspepsia, reflux
Atypical fractures - new onset hip, groin, thigh pain, jaw pain
Alendronic Acid (Bisphosphonates) - Medications which reduce absorption
Calcium salts
Food and drink
NSAIDs
ACE inhibitors
Increased risk of hyperkalaemia
Teratogenic (switch for Labetalol)
Tamoxifen
Increased risk of endometrial and ovarian cancer
Hot flushes
Increased VTE risk
CP450 inhibitor (can increase Warfarin efficacy)
Metformin
Lactic Acidosis
Does not cause hypoglycaemia
Sulfonylureas
Take in morning to reduce risk of nocturnal hypoglycaemia
Divide doses if total daily dose >160mg
SSRIs
6 weeks to see improvement
Dry mouth
Serotonin Syndrome = Agitation, pyrexia, hallucinations
Agranulocytosis - 3Cs
Carbimazole
Carbamazepine
Clozapine
(Azathioprine)
Azathioprine
Contraindicated in absent TPMT
Check TPMT activity
Methotrexate
Must be give alongside folic acid
Pregnancy - Safe Abx
Penicillins
Cephalosporins
Pregnancy - Abx to avoid in 1st trimester
Trimethoprim
Pregnancy - Abx to avoid in 3rd trimester
Nitrofurantoin
