Cardiac: 5-10mcg/minute
Vasoconstriction: 10-20mcg/minute

Adult: 0.5mg q3-5 minutes MAX 3mg
Pedi: 0.02mg/kg, MAX single dose 0.5mg, MIN single dose 0.1mg
diltiazem (Cardizem)
Adult: 0.25mg/kg IVP over 2 minutes, after 15 minutes may administer 0.35mg/kg
Adult Drip: 5-15mg/hour
Pedi: none
metoprolol (Lopressor)
Adult: 5mg SIVP q 2 as long as vitals remain stable MAX dose 15mg
lidocaine (Xylocaine)
Adult IVP: 1-1.5mg/kg, 0.5-0.75mg/kg q 5-10min MAX 3mg/kg
Drip: 2-4mg/min
Pedi IVP: 1mg/kg, repeat dose 0.5-1mg/kg
Pedi Drip: 20-50mcg/kg/min
amiodarone (Cordarone)
Adult VF/VT: 300mg IVP, may repeat 150mg IVP
Adult VT with pulse: 150mg over 10 minutes
Adult Drip: 1mg/minute over 6 hours, followed by 0.5mg/min for next 18 hours
Pedi VF/VT: 5mg/kg IVP may repeat twice MAX 15mg/kg or 300mg single dose
Pedi VT with Pulse 5mg/30 minutes
Pedi Drip: 0.005-0.01 mg/kg/min
naloxone (Narcan)
Adult: 0.4-2mg
Pedi <5y/o: 0.1mg/kg
albuterol sulfate (Proventil, Ventolin)
Adult/Pedi: 2.5mg in 2.5mL nebulized over 5-15 minutes
ipratropium bromide/albuterol sulfate (Combivent, DuoNeb)
Adult: 2.5mg albuterol, 500mcg ipratropium
Pedi: 2.5mg albuterol, 350-500mcg ipratropium
magnesium sulfate
Adult Respiratory: 2g over 2-5 minutes
Adult Cardiac: 1-2g in 100mL D5W or NS over 15 minutes
Adult OB: 4-6g over 30 minutes initial bolus; 1-2g/hour infusion
methylprednisolone (Solu-Medrol)
Adult: 125-250mg IV
Pedi: 30mg/kg IV
