2) he has consumed so much alcohol that the proportion of it in his breath, blood or urine exceed the prescribed limit
3) within 18 hours after the time of the accident he fails without reasonable excuse to provide a specimen for analysis
4) he fails without reasonable excuse to give his permission for a lab test of a specimen of blood taken from him
