Female, 15-35 y/o, butterfly facial erythema, mouth ulcers, pleuropericardial pain, polyarthritis, positive ANA, less commonly fever
SLE
Asymmetrical oligo or polyarthritis, DIP involvement, dactylitis (sausage fingers) of fingers and toes, psoriasis (maybe hidden), nail lesions, low back pain, morning stiffness
Psoriatic Arthritis
adolescent or young girl, fever, adenopathy, infectious contact or vaccination, exanthematous rash
associated GU/GI symptoms
Viral Arthritis
Acute onset, symmetrical, small and large joints, polyarthritis, urticaria, macular rash, erythematous tender skin nodules
Viral Hepatitis Polyarthritis
low grade fever, uticaria, lymphadenopathy, large and medium size joints, drug exposure (sulfa, penicillin)
-Dramatically ill even without lab values
Serum Sickness
preceding or synchronous GI symptoms, oligo or polyarthritis of lower limb, spondylitis, erythema nodosum
Inflammatory Bowel Disease Arthritis
Male, large joint arthritis, urethritis, conjunctivitis, painless mouth ulcers, circinate balantis (dermatitis of glans penis), cutaneous lesions of palms and soles, nail changes similar to psoriasis
History of chlamydia, bloody diarrhea, food poisoning
Reiters symdrome or Reactive Arthritis
Spiking quotidian fever (reoccurs everyday), polyarthritis, salmon colored macular or maculopapular rash, fatigue, malaise, weight loss and myalgia
Adult onset Still’s disease
Endemic area for deer ticks, erythema chronicum migrans (bulls eye rash), history of neuro symptoms such as Bells palsy, cardiac arrhythmia, tendinitis, bursitis followed by asymmetric large joint
Lyme Disease
Female, 15-35 y/o, butterfly facial erythema, mouth ulcers, pleuropericardial pain, polyarthritis, positive ANA, less commonly fever
SLE

Psoriatic Arthritis
less than 50 y/o, fatigue, weight loss, severe morning stiffness, pain and stiffness of shoulder and pelvic girdle, rapid response to steroids
Polymyalgia rheumatica
polyarthritis or polyarthralgia less than 40 y/o, female, systemic malaise, Raynouds, skin rash, dry eyes/mouth, digital ulcers, sclerodactyly, proximal muscle weakness, postive ANA
