Mental illness epidemiology
-41 % of those with mental illness receive treatment
-50% are compliat
reason:
-stigma
-affordability
-shortage of psychiatrists
-incorrect diagnoses
history taking of depression
check for: -mood changes sleep changes indication for mental health screening: high symptom count high severity of symptom symptoms greater than 6 weeks recent stressor
depression diagnosis
REMEMBER to ask about suicidal ideations
-if they have a plan that means they are actively suicidal and are an emergency
Major depressive disorder
five or more of the following symptom present for atleast a 2 week period and atleast one symptoms is either depressed mood or loss of interest:
- depressed mood
- diminished interest
- change in sleep
- fatigue
- decreased ability to concentrate
cannot be due to other causes
physical complaints of those with depression
-gi-vague,nausea,vomiting,change in appetite cardiac-vague chest discomfort sleep-changes in sleep pulmonary-shortness of breath headaches neurological-difficulty concentrating social-loss in interest
anxiety patient physical symptoms-
-excessive worrying
-appetite changes
-sleep changes
-somatic somplaints- palipitations
cognitive changes
anxiety history taking
Ask:
- over the last two weeks have you been feeling anxious
- have you been unable to control worrying
- use screening tool Prime -MD and GAD-7
5- mild symptoms
10-medium symptoms
15-severe sympoms
diagnosing anxiety disorder
- excessive anxiety and worry happening for atleast 6 months
- assx with three or more of the following symptoms:
- restlessness
- easily fatigued
- irritability
- muscle tension
- sleep disturbances
- difficulty concentrating
-not better explained by another disorder
somatic symptoms of anxiety
cardiac-i think im having a heart attack'gi-change in weight and appetite pulmoary -shortness in breath -headaches loss of focusing social-isolating
rule out for mental illness
-important to do medical workup to rule out other things that could rule out other possibilities
also do a physical exam:
for example depression can be linked to thyroid disease and anxiety can be linked to cardiac disease
routine diagnostic tests to include
-cbc
CMP
-urinanalysis
-thyroid function
treatment plan
1/medication management
- psychotherapy
- clinical coordination
