Indications for the use of methotrexate?
Immunosuppression and dance chemotherapy
MOA methotrexate
Methotrexate is an antifolate which antagonises Dihydrofolate reductase. This inhibits purine and thymidine synthesis, inhibiting DNA synthesis.
Methotrexate ADRs
Mucositis, myelosuppression, hepatitis and cirrhosis, increased infection risk and teratogenic
DDIs methotrexate
Any drugs that affect renal blood flow and renal elimination, e.g NSAIDs
Indications for use of Sulfasalazine?
Rheumatoid arthritis and IBD
MOA of sulfasalazine?
Metabolised to active component 5ASA. Inhibition of T cell proliferation and IL2 production. Reduced chemotaxis and degranulation.
ADRs of sulfasalazine?
Myelosuppression, hepatitis, rash and gi disturbances
Contraindications of sulfasalazine?
Renal impairment and hypersensitivity
What class of drug does sulfasalazine belong to?
Aminosalicylates
Examples of Anti-TNF agents?
Infliximab, rituximab, adalimumab
MOA anti-TNF alpha agents
Blocks TNF alpha, decreases inflammation, decreases angiogenesis, decreases joint destruction
ADRs of anti TNF alpha drugs
Increased infections, tiredness, dizziness, itching and GI disturbance.
