4) WHEN and WHERE the work will be accomplished, and by whom
5) Objective Quality Evidence (OQE), including new material acceptance and in-process testing and results
6) Compliance with applicable specifications, drawings, procedures, and instructions
How long are REC packages maintained for?
For the life of the system
What is the highest pressure that can exist in a system or subsystem under normal operating conditions?
(determined by the source)
Max Operating Pressure (MOP)
What is the approximate pressure a system operates while preforming its normal function?
(this pressure is used for system identification)
Normal Operating Pressure (NOP)
What is the maximum pressure that can exist in a system or subsystem during any condition?
(determined by relief valve setting)
Maximum System Pressure (10% over MOP)
What are the types of boundaries in a REC?
1) Work
2) Test
3) Isolation
What block are boundaries located in a REC form?
Block 9
What are work boundaries?
Work boundaries define the limits to which work was preformed.
Enter the FIRST stop valve upstream and downstream of the joint/component being entered.
What are test boundaries?
Test boundaries define the limits for testing to certify the work.
For LOW pressure systems, enter the FIRST stop valve upstream and downstream. For HIGH pressure systems, enter the FIRST AND SECOND valves upstream and downstream of the joint/component being entered.
What are isolation boundaries?
Enter the valves, switches or other components that must be verified as being open, shut, re-energized, etc.
Include the required position of each isolation component.
Ex. AHP-V-627 (SHUT)
What is a REC log?
A chronological list of all RECs including the REC number, system, open and close dates.
What is a REC form?
Authorizes a re-entry into a certified system. A completed form certifies the system is safe for manned use.
What is REC continuation page?
May be used to record supplemental data, references, parts/materials or procedures for a REC package.
What is a REC continuation page Rework?
Describes the reason for rework, and lists what steps of the procedure which will be repeated.
What are Test and Inspection Reports?
Used for operational testing, shop inspections, cleanliness inspections and other tests and inspections deemed necessary.
What are the types of Test and Inspection reports?
1) Joint Tightness Test
2) Valve Seat Tightness Test
