Hydrochlorothiazide
A
Acts on DCT, inhibits NaCl co-transporter, reducing Na+ re-absorption, increasing Na+ and K+ loss, leading to reduced peripheral vascular resistance.
Used to treat hypertension, adjunct to treat oedema (secondary to CCF, cirrhosis, CKD)
Half life 6-12hrs. Onset 2hrs, peaks 4hrs.
Dose 12.5-25mg daily reduced sBP by 5-7mmHg.
Renal excretion.
AE’s: exacerbation of SLE, hyperuricemia leading to gout, hyperglycemia (in pts with latent diabetes), hypotension, eletrolyte loss
