Explain HIT?
Heparin Induced Thrombocytopenia. Heparin binds the inhibitor of antithrombin causing increased AT activity. This can lead to platelet destruction where the fragments cause thrombosis of the remaining platelets
What is DIC?
pathological activation of the coagulation cascade
What are the lab findings of DIC?
decreased platelets increased PT/ PTT decreased fibrinogen microangiopathic hemolytic anemia elevated fibrin split products (D dimer)
What causes DIC?
sepsis obstetric complication adenocarcinoma APL (acute promyelotic anemia) Rattlesnake venom
What converts plasmniogen to plasmin in fibrinolysis?
tPA, tissue plasminogen activator
What does plasmin cleave?
fibrin, fibrinogen, coag factors
What cleaves plasmin?
alpha2 antiplasmin
what are the lab findings of a fibrinolysis disorder?
increased PT/PTT (destroyed factors)
increased bleeding time
increased d dimer
what is aminocaproic acid used to treat?
Fibrinolysis disorders. blocks activation of plasminogen
