What to do during RBC Spillover
-check for signs of hemolysis
-lead or higher verify
-if there is no hemolysis, place on last return
-if return is successful, donor is put on 56 day watch
-if they have a second rbc loss in 56 days, they will be deferred for 56 days
-give Gatorade and have donor wait 5 minutes
-take Machine out of service
What to do if there is a bowl break
-determine the reason and have management verify
-check for signs of hemolysis
-end the procedure
-check the plasma valve, if it is unlocked, the bottle is air contaminated
-inform the donor of the rbc loss
-follow the steps for not receiving saline
-log defect if applicable
What to do during a citrate reaction
Slow the return, reassure donor, slowly turn up return if they feel better
Symptoms of a citrate reaction that DO NOT require CMS
Metallic taste in mouth, tingling/numbness in hands/feet/lips
Symptoms of citrate reaction that DO require CMS
Tingling/numbness past the extremities, muscle tightness, convulsions, symptoms last more than 10 minutes
What to do during hypotensive reaction
Place donor in return, elevate their feet, fan them off, talk and reassure them to reduce anxiety, place ice pack on chest/behind neck
Symptoms of hypotensive reaction that DO NOT require CMS
Pale, clammy, lightheaded, dizzy, auditory or visual disturbances, nausea, feeling hot
Symptoms of hypotensive reaction that DO require CMS
Vomiting, passing out, chest pain, bluish tint to skin, symptoms lasting longer than 10 minutes
What to do during hemolysis reaction
Stop the procedure and notify management and CMS. Contact EMS. Take machine out of service
What to do during a power outage
-turn off all machines
-hold down donor valve and turn blood pump towards donor 5 times to prevent clotting
-if power does not come back in 5 minutes, perform gravity infusion
-if infusion is successful, give donor saline and have them wait 5 minutes
-if all blood cannot be returned, follow saline steps and inform donor of RBC loss deferral period of 56 days
What to do during wet DPM
-perform gravity infusion
-give gatorade
-clean dpm with wet tech wipe
What to do during ac runout
-end procedure
-place donor in last return if possible
-gravity infuse if not
-counsel donor on hydration
-if donor doesn’t receive saline, give gatorade
