A
- reduce length of stay in hospital

- improve community services coordinations post discharge

What is the home first approach during discharge?
A
- encourages early discharge from the hospital to prevent decline in mobility + independence
- focuses on helping patients transition home with adequate support + improving QoL whilst reducing pressure on hospital resources

What do discharges involve?
A
- medication to take home (TTO)
- transport
- therapy assessment: is ongoing OT/PT required?
- restarting package of care
- OP appointments
- district nurse/palliative care referral if needed

What goes wrong in a discharge plan/generally that causes re-admissions?
A
- inability to obtain suitable package of care
- patient health complications
- communication issues between social services + health care professionals
- family decisions
- issues with funding
