heart failure (HF) with cardiogenic shock

Dobutamine β1 > β2, α
Unstable bradycardia, shock; inotropic and
chronotropic effects at lower doses via β effects;
vasoconstriction at high doses via α effects
Dopamine D1 = D2 > β > α
Anaphylaxis, asthma, shock, open-angle
glaucoma; α effects predominate at high
doses. Stronger effect at β2-receptor than
norepinephrine.
Epinephrine β > α
Postoperative hypertension, hypertensive crisis.
Vasodilator (coronary, peripheral, renal, and
splanchnic). Promotes natriuresis. Can cause
hypotension, tachycardia, flushing, headache
Fenoldopam D1
Electrophysiologic evaluation of
tachyarrhythmias. Can worsen ischemia. Has
negligible α effect.
Isoproterenol β1 = β2
Autonomic insufficiency and postural
hypotension. May exacerbate supine
hypertension
Midodrine α1
Urinary urgency or incontinence or overactive
bladder. Think “mirab3gron.”
Mirabegron β3
Hypotension, septic shock
Norepinephrine α1 > α2 > β1
Hypotension (vasoconstrictor), ocular procedures
(mydriatic), rhinitis (decongestant), ischemic
priapism
Phenylephrine α1 > α2
