Reliability
The reliability is in the creation phase, and it is reliable in the sense that only one type of person can write a record. For example, an engineer can write a reliable blueprint or a doctor can write a diagnosis. This then goes into the completeness of the record, having the document signed off by someone who has the authority to do so.
Accuracy
The accuracy component of the record has to do with precise, correct, truthful and pertinent content of the record.
Authenticity
The authenticity of the record is if the record has integrity and if the record is the record that it claims to be (the records identity)
