Q

Compare and Contrast Osmotic vs. Secretory Diarrhoea

A

Osmotic is low volume, (<200mL/day) low Na (<70mEq/Litre), high osmotic gap (as in > (cations x 2), as in > (Na + K) x 2), acidic, with present reducing substances - all about CHO malabsoprtion/digestion an - stops with enteral feeds.
STOPS when oral intake is stopped

VS,

Secretiory - which is a channelopathy - excess electrolyte flux into lumen
Bacterial toxins (Giardia, Cryptosporidium, Rota, ETEC, Clostridium perf)
(Knock off of Villi)
Congenital electrolyte transport deficience
Phaeochromocytoma
Mastocytosis
Hormone secreating tumours
Or Microvillus incluison disease, tufting enteropathy

CONTINUES despite cease feeds
No reducing substances
High Na > 90
PH >6
Volume > 200mL/day
