Osmotic diarrhoea
When solutes left in gut lumen. Could be due to poorly absorbed solute e.g. sorbitol OR malabsorption e.g. lactose intolerance where lactose remains in the lumen. This draws h2o out of ECF into gut lumen to equalise osmolarity.
Stops when fasted
Secretory diarrhoea
Abnormal secretion of h2o + salts in LI. Absorption of Na is impaired by villi + secretion of Cl from crypts is increased e.g. cholera - toxin - activates adenyl cyclase - increased cAMP - Cl channels open - h2o + Cl leave cell into gut lumen
Treatment
ORS - based on principle that absorption of Na (therefore h2o) is enhanced by active absorption of glucose. Uses glucose linked Na transport as this still works when other pathways may not.
