How does E. coli cause HUS?

A

HUS = simultaneous occurence of microangiopathic hemolytic anemia, thrombocytopenia, & AKI

Shiga toxin binds to receptors on glomerular and renal tubular epithelial and endothelial cells (also in pancreatic cells) → toxin-mediated injury of epithelial and endothelial cells → microvascular damage that promotes local thrombus formation and systemic thrombocytopenia → RBC shearing when they encounter microthrombi → microangiogenic hemolytic anemia → tissue ischemia and necrosis → acute kidney injury
