Disruptive Mood Dysregulation Disorder (DMDD)
- Symptoms must have started before 10, but can be dx between 6-18 years old.
- Symptoms in at LEAST 2 settings.
- Outbursts => 3 per week and inconsistent with developmental level.
- Symptoms => 1 year with no more than 3 months without symptoms.
NO episodes meeting full criteria for manic/hypomanic episode lasting longer than a day.
Behavior does NOT occur during MDD.
Manic vs. hypomanic episode
Manic
- 1 week unless hospitalized
- Marked impairment in social or occupational functioning or hospitalization necessary
- May have PSYCHOTIC features –> manic by definition
Hypomanic
- => 4 consecutive days
- Symptoms not severe enough to caused marked impairment or necessitate hospitalization
- NO psychotic features
Signs of PCP intoxication?
A dissociative anesthetic that causes:
- Hallucinations
- Dissociative feelings,
- Agitation, many cases can lead to psychotic/violent behavior
- Confusion
- Tachycardia
- Nystagmus (horizontal, vertical, rotary)
- High doses can cause HTN, seizures, rigidity, life-threatening hyperthermia
TREAT WITH BENZOS FOR SEVERE PSYCHOMOTOR AGITATION
Signs of MDMA intoxication?
- Euphoria
- Increased energy
- +/- bruxism, hyperthermia
VIOLENT AGITATION NOT COMMONLY SEEN.
Signs of LSD and psilocybin intoxication?
Hallucinogens that cause:
- VH and illusions
- Euphoria or dysphoria
- Depersonalization
PTSD vs. ASD
PTSD:
- Trauma occurred at any time in past
- Symptoms > 1 month
ASD:
- Trauma within 1 month of symptoms
- Symptoms < 1 month
Separation Anxiety Disorder timeline for diagnosis
Symptoms => 1 month in children
Symptoms => 6 months in adults
During normal development:
- Stranger anxiety begins ~ 6 months with peak at 9 months
- Separation anxiety begins ~ 1 year with peak at 1.5 years
Persistent complex bereavement disorder (complicated grief)
Persistent yearning for deceased and prolonged emotional pain related to the loss, accompanied by maladaptive ruminative thoughts and dysfunctional behaviors.
REM sleep behavior disorder?
Violent movements during sleep in response to dreams, often of fighting. PRONOUNCED anti-psychotic sensitivity.
