Two clinical clues to help diagnose joint problem
- Joint pattern
2. Presence or absence of extra-articular manifestations
morning stiffness - after prolonged period of rest.
Yes does not confirm inflammation. Also ask how long stiffness lasts.
- move knee couple of times then ok this is not inflammation
- Pt. says stiff until lunch then there is inflammation of joint receptors
Regular Osteoarthritis length of stiffness
No inflamation or little. Stiffness will less than 15-30 min
Rheumatoid arthritis length of stiffness
stiffness can last 2-3 hours depending on severity
One joint involved in toe
Gout - monoarticular disease
Polyarticular disease that is symmetrical
RA - if hand affected then usually both are affected
if you have DIP joint involvement what type of arthritis is this
OA
If you have metacarpal involvement what type of arthritis is this
RA
If patient comes in with fever
Could be gout or may have infection of joint
If patient has a rash
SLE (face), Psoriatic arthritis
If patient has nodules on the body surface
RA
2. Also seen in gout. tophi
Neuropathy and joint problem
could be vascularitis called polyarteritis
Example of inflammatory bowel diseases
- Chrones disease
2. Ulcerative colitis
Calcium Pyrophosphate
Pseudogout.. Positively birefringent
