Diagnosis Flashcards
Do you measure BP in both arms?
Yes
What difference in BP would prompt you to recheck the readings?
15mmHg
If the readings remain Higher in one are than the other which arm would you proceed to take the readings from?
The arm with the higher reading
When would you use a manual sphygmomanometer?
When the patient has an irregular pulse for example with AF
If the patient is suspected to have postural hypotension, how would you go about their BP check?
- Check the BP sitting/lying
- Ask the pt to stand for 1 minute
- Repeat the reading
What reading drop would you be expecting in someone with postural hypotension when comparing LBP and SBP?
> 20mmHg drop
What BP reading would prompt you to take a second reading in the clinical setting?
> 140/90 mmHg
If you get 3 consistently differing BP measurements, which 2 readings average would you use for the clinic BP?
The last 2 recorded
At what reading would you offer a patient ABPM?
If the patients BP lies between 140/90mmHg and 180/120mmHg
How many measurements should be taken every hour on ABPM?
A minimum of 2 per hour
How many readings are needed to take an average for the diagnosis of HTN on ABPM?
14 readings
When would you refer a patient for a same day specialist assessment?
When their BP is 180/120mmHg< and there are the following signs;
- Retinal haemorrhage and/or papilloedema
- Life threatening symptoms such as;
+ New onset confusion
+ CP
+ Signs of HF
+ AKI
What would you do if there were signs of end organ damage?
Consider starting an antihypertensive immediately without the result of ABPM or HBPM
