GLP 1 RA once weekly and short acting
SHORT: exenatide, liraglutide, lixisenatide
ONCE a week: exenatide ER, dulaglutide, semaglutide
GLP 1 RA AE
-Nausea, vomiting, diarrhea
-Dehydration (decreased appetite, dyspepsia, dizziness, headache, jitteriness)
-Delayed gastric emptying effects
Caution: pancreatitis risk, gallbladder, AKI
Exenatide (Byetta®)
inject SQ within 1 hr before a meal TWICE a day
skip any missed dose, no doubling
Liraglutide (Victoza)
-0.6 mg daily for 1 week, then 1.2 daily for 1 week, then 1.8 daily
Saxenda (liraglutide FDA approved for weight loss)
-0.6 daily, increase by 0.6 each week until 3 max
Lixisenatide (adlyxin)
20 mcg daily
-HS rxn, anaphylaxis
Bydureon Bcise Pen (Exenatide ER)- Administration
-remove from fridge 15 min before use
-shake 15 times
-cloudy, no particles
-once a wk inj
Dulaglutide (Trulicity)
0.75 mg sc once a week, then 1.5 mg
missed dose: give if within 3 days
warning: Diabetic Retinopathy Complication
Once Weekly Semaglutide (Ozempic)
-Start with 0.25 mg once weekly x4 weeks
-Then 1 mg once weekly for 4 weeks then 2 if needed
Avoid in patients with retinopathy or high risk for retinopathy
Semaglutide Injectable: Additional FDA Approval for Weight Loss (Wegovy)
-Titrate over 16-20 weeks up to 2.4 mg once a week
Tirzepatide (Mounjaro)
-2.5 mg once a week for 4 weeks
-Increase by 2.5 increments every 4 weeks until max of 15
IV Infusion: Teplizumab (Tzield)
-over 30 min, once daily for 14 days
AE: low WBC, rash, HA, infection, cytokine syndrome
-childhood vaccines must be administered prior to use, avoid live/inactivated vaccines
