Site and mechanism of action
Pancreas - stimulates prolonged insulin secretion
DPP4i
Site and mechanism of action
Inhibit DPP4 = increased concentration of active incretins = pancreas -increased insulin secretion and reduced glucagon production
SGLT2i
Site and mechanism of action
Kidneys - reduces reabsorption and increases urinary secretion of glucose
Biguanide
Site and mechanism of action
Increases hepatic and peripheral tissue sensitivity to insulin
Liver - reduces hepatic gluconeogenesis.
Peripheral tissue - increases utilisation of glucose
GLP1 receptor analogues
Site and mechanism of action
Pancreas - increase insulin secretion and reduces glucagon secretion.
Stomach - delays gastric emptying which slows glucose absorption
Thiazolidinedone (1)
Names and dose
Pioglitazone 15-30mg od, 45mg after 6- 8weeks
Thiazolidinedone site and mechanism of action
Peripheral tissues - increases sensitivity to insulin
Liver - decreases hepatic glucose output
