Design 3 Flashcards
4 functions of major connector
- vertical support / soft tissue protection
- indirect retention
- promote patient comfort
- linking of saddle areas
5 options of major connector in maxilla
- ring
- horseshoe
- palatal strap
- palatal bar (rare)
- palatal coverage
when to use a ring
multiple bounded saddles
good option. lots of tooth support –>ring –> can feel temp of food on palate
advantage of horseshoe denture 5
- strong
- useful for missing anterior teeth (aesthetics?)
- relieves palate
- gains some support from tissues
- good to pts with strong gag reflex
disadvantages of horseshoe denture4
- can deflect and distort in free end saddles
- bulk of metal on anterior border
- no suction from palate
- not bulky –> not v rigid
advantages of palatal strap/bar 4
- thin and versatile
- can be in 2 planes increasing rigidity
- well tolerated by pts
- inc surface area –> good tissue support
disadvantages of palatal strap 2
- must be at least 8mm wide for strength
- pts can be sensitive to strap border positioning
ideal position of palatal strap
posterior to rugae, anterior to hard-soft palate border
disadvantages of palatal bar 3
- bulky
- narrow –> little support from palatal tissues
- if anterior to second premolar region may impede speech
are palatal bars/straps more common? when is other used
straps more common (thinner, more tolerable)
bar used if pt already has bar
advantage of complete palate denture 2
- ultimate rigidity and support
- most comfortable
disadvantages of complete palate denture 3
- extensive coverage of tissues (no free palate)
- oral hygiene, denture hygiene issues
- difficult to adjust/get well-fitted coverage of palate
rules for borders of maxillary major connectors
- must be at least 3mm from gingival margin or extended onto palatal tooth surfaces
- should blend with anatomy
- borders that contact soft tissue should be beaded –> less distinct as approach gingivae
what maxillary major connector to use with good/bad perio support
good: palatal strap (/bar)
bad: more tissue support (palatal coverage, ring etc)
3 possible max major connectors when anterior teeth need replacing
- ring
- complete palate
- horseshoe
