within normal limits
gold standard of diagnosis
biopsy with microscopic examination
color change, no elevation
macule
raised lesion less than 5mm
papule
raised lesion more than 5 mm
nodule
narrow groove

ecchymosis
abnormally swollen, distended or dialated
varicose
flakes of retained surface of keratin
scale
dried blood, serum, or purulent exudate on the skin surface
crust
superficial, partial or total loss of surface epithelium; may arise secondarily form rupture of bulla (blister)
erosive
loss of surface epithelium down to the connective tissue; often appears depressed
ulcerated
exhibiting numerous surface projections
papillary
exhibiting a rough, watery surface
verrucous
base is the widest part of the lesion
sessile
base is narrower than the widest part of the lesion
pedunculated
red in color due to dilation and congestion of capillaries, increased blood flow. implies injury, infection, or inflammatory process
erythematous
fluid filled, elevated, superficial, 5mm or less in diameter
vesicle
fluid filled, elevated, superficial, greater than 5mm in diameter
bulla
superficial, circumscribed cavity filled with pus (purulent exudate)
