Q

Name three vesiculobullous conditions and brief features

A

Bullous pemphigoid - often seen in elderly requiring admission.
- flexural surfaces-
accumulation of fluid between epidermis and dermis
- infiltrate contains eosinophils - release elastases that destroy anchoring filaments
- IgG and c3 attacking dermoepidermal junction - destroys anchoring filaments

Pemphigus vulgaris
destroys adhesion molecules which keep keratinocytes - free floating in the epidermis

Pemphigus foliaceus
very rare
antigen desmoglian - keeps keratinocytes
stratum corneum destroyed - subcorneal blisters
shows inflammatory infiltrate
