MRI important to check for patchy inflammation to know where need to do the muscle biopsy
But Ultrasound is used more IRL because MRI is too expensive
Perifascicular Atrophy
Consider dermatomyositis
First Line treatment for dermatomyositis
Prednisone (but irl they give it with MTX - but if sagot sa exam prednisone)
Usual muscle involvement in steroid induced muscle weakness
most common are the hip flexor muscles
Second line treatment for dermatomyositis
METHOTREXATE but rule out viable pregnancy first
Starting at 6 months of juvenile dermatomyositis MTX is used
They take it without steroids compared to adults who take MTX with STEROIDs together??? (double check nalang)
Dermatomyositis usually involves which muscles
Starts the proximal and then the distal but usually more on proximal lang affected
